About you

Last four only. Never write your full Social Security number in a dispute letter, and do not type it here — this field accepts four digits.

Who you are writing to

The credit bureau, or the company that reported the item.

For a direct dispute, use the address the furnisher specifies for disputes, not the payment address.

The item you are disputing

Partial is fine and safer. Use the masked form the report prints.

Quote it. "Reported as 120 days past due, balance $4,318, status: charge-off."

Facts only, in your own words. State what you know to be true, not what you suspect.

"Delete this account," "correct the balance to $0," "remove the late payment reported for March 2025."
